1. Name you wish to be called on the zine to give credit

2. What sort of art you are going to be using. (Options visual (drawing/digital), writing, photography, or poetry)

3. Who will be in your picture (if you have a ship name please say which)

4. for which show/set (Crossovers are excepted) 

5. Way of contact such as email 

6. social media usernames for artist profiles so people may see the rest of your works.

Please fill out and send it to Willow_Tracy33@outlook.com before Feb 10th. you'll have till Feb 13 to have the art ready so I can put it into the zine
